Case Management


JFS offers case management as an opportunity to enable more support systems for older adults in their homes. Case Managers assist in connecting individuals to community resources, facilitating family discussions regarding change in care for the individual, maintaining financial stability, reviewing eligibility for government programs (Medicaid, Social Security, Veteran’s Benefits, food stamps, nursing home diversion waiver, etc.), and arranging services that will allow older adults to feel safe, comfortable, and independent in their home, as well as give family members peace of mind.  We provide case management services to survivors of the Holocaust and their caregivers through partnerships with Rockland JFS, Jewish Federation of Greater Orange County.
